PLACEHOLDER cutoffs — NOT calibrated. With only CMS QM + staffing stars and an open posting (the only inputs populated for the current 576-prospect NY cohort), the max reachable score is ~64, so Tier-1 (>=70) is unreachable until the F641 / leadership-turnover / Medicaid-% / CCN backfill (ADR-0040 design §3-4) lands. Phase 4 MUST, on the first full score pass, derive tier1/tier2 from the cohort distribution (percentiles for the ~15 Tier-1 / ~60 Tier-2 targets) and pass them via the cutoffs param rather than relying on these defaults.
